An original quadruple form must be used. Parents should complete the following fields on the top of an original form: School, School Year, and Instrument Type. Then read the six contract stipulations listed. Finally, complete the fields at the bottom of an original form: Student's Name, Grade, Home Address, Phone, Parent's/Guardian's Name, Signatures, and Date. The white/top copy of the form will be returned to you (with a receipt) when we receive payment of the $20.00 rental fee (checks payable to IRMS or cash).
